I think there is a checkbox under DICOM/local AE called SR coversion. Try to turn it on , then save the settings. Restart the system and test with new paitent.

Thanks! It also stopped sending dynamic images. Only send the stills. Any ideas?

 The Toshiba SR might not be supported by the DICOM viewer (depending on which one he is using). Instead save the structured report as a still image and it will send to DICOM the same as any other still image.

 In regards to the "dynamic images"... I can only assume he is referring to a cine image. That would probably be something misconfigured in PIMS. Press PIMS on the touch screen to check and make sure the assigned save key is set to saving the proper image type. Also press Menu, System Setting, Exam Type, click on the Application Preset Editor drop down menu, scroll to the bottom, press Shift+<, click on UserFunction, and click on OUTPUT. This will enable you to press the the assigned save hard key on the lower control panel and double check it is set to the right save function. 

 If all this is correct and it is still not working properly, or if the original settings somehow mysteriously changed on it's own, reload the software and restore from the last B/U disk. It might be a good idea to make a a B/U disk right before the SW reload in case the old one got damaged somehow.

Thanks Mike. We added a destination it was sending fine. It still send correctly to the original destination but I tried turning on Sr and not it will not send the dynamic runs to the new destination. It's stays in active status on the machine.
