venous pressure errors

We are experiencing intermittent venous pressure errors on multiple machines in our organization with multiple users. We are starting to track procedure information to determine if it is a patient type or procedure type issue verses machine.

Any one else experiencing the same venous pressure intermittent issues? Any ideas on potential causes?

Recently I had a machine with an intermittent problem like this.  I realized the T1T A/V calibrations were out of range.  Make sure the sensors are within specs/calibrated first.  It was a bearcat to calibrate T1T values, but once done, it was resolved.

Our hospital was also having this same issue. I did have one machine that needed calibration but when the other machines had issues all inspections showed within specs. We believe it is a blood cartridge manufacturing issue. 

The Dialysis director reached out to Gambro and have sent in failed blood cartridge sets. When our director reached out again with second round of issues, they returned all the cartridges in that lot number. Apparently no questions asked. We have not had issues in the last 2 weeks.

Thank you!   We have finally worked our way out of it as well.   It was pointing to either the tubing or dialyzer on our end.   That was a long haul but we haven't had an issue in almost 2 weeks.
