Does anyone have the scoring information to Risk Factor medical equipment to come up with the "Risk Score"

Alexandra, I used a standard grading system I think it consisted of three questions with number values, you had a number value dependent how you answered the question add up all 3 values and it gave you the total risk factor of that equipment. I am looking for that grading system.
