Possible database corruption.

Probably a corrupt study on the hard drive.

Deleting old studies in regular intervals is recommended.

If this does not go away after clearing the patient data, then the hard drives will need to be replaced and software will need to be reloaded.

This could also be the ID chip.  If it was unplugged or removed, then you would get a 511.
