The problem is in the front end PCB or Main PCB. The front end PCB is a common failure on these devices. I would open the unit and check/wiggle/push all connections on Front End PCB and Main PCB, make sure chips are pushed into sockets and check it again.
If failes, then check front end pcb reference voltage. From service manual below.
Reference Voltage Check
1. Refer to the calibration test point locations in FIGURE 4-15 on page 4-29. Connect a
DVM between TP22 and TP33 (GND_A) on the Front End board.
2. Adjust R253 on the Front End board for 5V ±0.1V.
If that does not work, swap front end PCB to get problem down to board level. Front end PCB is easy to replace, but you will need to do some calibrations after. At least swap it out and see if error goes away. Denova Medical has front end PCB for sale.
