SYSTEM ERROR 28 ! fault code on screen after trying to take an image

Hello all, We are a bit desperate for information about our Lorad M-IV mammogram machine. 

We have 3 in total for NDT on our carbon fiber parts: 1 is in storage due to tripping main breaker the minute you start it up (still unsure why), 1 has communication issues with our digital detector under it (software?), but the one that does work has now a SYSTEM ERROR 28 ! code showing with every image you try to take. The settings haven't been altered and are always set the same way. 

But this morning it came with this fault, and after resetting it multiple times and/or powering it down for a bit, it still comes back. we have an installation manual and operator manual, but none of them have any fault codes or troubleshooting in them about what to do... any ideas here?

Tengo un problema con el hologic selenia, me arroja el mismo problema  28,  estoy trabajando en el,  es probable sea la tarjeta del filamento.....

Error 28 es sobre voltaje en el filamento,  deberia ajustarse con algunos potenciometro en la tarjeta,  debes hacer una prueba cambiar a filamento  pequeño en modo manual y realizar un disparo


Translation:

Error 28 is over voltage in the filament, it should be adjusted with some potentiometer on the card, you should do a test, change to small filament in manual mode and perform a trip

That is an easy answer, but now finally found the issue, and its not the tube...
